Maybe someone with an accent (Hispanic maybe) tried to say Rick, but since r's come out as d sounds in Spanish, they said Dick and people misunderstood and thought it was a nickname...

Someone probably accidently said "Rick-Hard" and then someone with a nasty mind said "Dick-Hard!" And from that point on, everyone called Richard, Dick. Because 'Dick Hard' was just too long. (No pun intended)
