A lot of nicknames like this (Dick, Billy, etc.) came about when names were translated into different languages. Dick is thought to have come about when Richard was introduced to the English by the Norman French.

1973- the young Richard Lee was diagnosed with micropenis. After years of being made fun of, Richard finally got penis enlargement in order to put a stop to such ridiculous bullying. But the operation went horribly wrong, and poor Richard's penis was now too large. People continued to tease him, and therefore, the name "Dick" was born.
