I was thinking of making a post about this. You beat me to it. It always pisses me the hell off when a huge chunk of plot in a movie or tv show is there solely because, not just of an ordinary misunderstanding, but because a character, with zero explanation, decides not to take all of 10 seconds to clarify something to another character. For example, Character 1 will have witnessed Character 2 doing something bad in person. Character 1 will then go to Character 3 and goes about telling 3 that 2 is bad. 3 will then immediately not believe 1, claiming that 1 is either jealous of 2 or is simply mistaken. 3 will then walk away and 1 will just let it happen without telling 3 the crucial point that they aren't just suspicious and actually saw 2 doing something bad with their own eyes. Why in the hell would 1 not reveal this when it would be easily the best way to get 3 to believe them? There's no logical reason!

2 is the bad guy, 1 is the guy who witnessed 2 being bad and 3 is the character that won't believe that 2 is a bad guy.
