Also, someone is always getting married, and when they say "speak now or forever hold your piece" in the wedding ceremony, someone WILL stand up and object. Oh, and there are a whole lotta DNA tests and switching of results.

And there is only ever one doctor, who just happens to be an attending/consultant of all specialties in existence. And people don't look outside their own back yard for new relationships; they simply get it on with whoever their neighbours are. And weddings are a sure-fire way of killing someone off.
