Also, I find it very difficult to eat the middle without eating the outside first. I'm not sure how you would do that...

Eating the cookie from top to bottom. Therefore, you would be eating outside, middle and then you'd be eating more outside last. If you go around the outside of the cookie and chew off all of the outside first, then you're eating the outside before the middle. Also, if you break the cookie in half, you can eat the entire middle without eating any of the outside at all.
