I'd imagine that the person's own body heat would keep the lip ring relatively warm.

The body heat does keep it warm. And I know because I've kissed people with piercings and I used to have mine done.

Some metal is only cold because it is good at conducting heat, and therefore will make the temperature difference between your skin and the metal more noticeable. Following that, though, it will also absorb heat from the person's lip and not feel cold upon contact with your lip. Having kissed and received head from a person with a lip piercing, I can tell you first-hand that it is no different at all, and you even forget they have the piercing after a while.
