It's not normally something I refer to so I can't say for sure, but usually the names of anything I begin with a capital letter.
I think I do. I tend to do it with the titles of any books.
"Bible" just means "book". You don't need to be afraid. God is not going to strike you with lightning because you misspelled the name of His book. It isn't even an actual name, it's just that we don't have any other word to call it. Except "scriptures", which means "writings".
It deserves respect.
