Not really, though. I use the phrase when I know what I say can be interpreted as offensive, but I want to make it clear that that is not my intention and I'm simply bad with words sometimes.

I'd have to disagree. I've had others say those words to me and what was said wasn't remotely offensive. Some people just use the phrase to ensure others don't misunderstand their intent, or to try to lessen the blow of something they're concerned will be taken as offensive.
