Oh yes that happened to me on my english final! She gave us a page from the final and the answers so I remembered the letters, but was off by 1 and messed everything up..

That's why I get all pessimistic before I get my grades back, so I can be pleasantly surprised that I didn't get as low as I thought.

Or when a teacher is supposed to explain what to do as there are no instructions on the sheet, but the teacher is a substitute and just tells us to do the test and the whole class fails because we thought we were supposed to answer all the questions and not just answer one in essay form.
