Usually, when someone mispronounce a word it's because they've only read it. Which I think is a good thing.

Absolutely. For instance, I can read the word Worcestershire just fine. Pronouncing the word, on the other hand, is difficult for me to do.

Absolutely. For example, I used to mispronounce a name of a writer simply because I haven't discussed him with anyone and haven't listened to any podcasts about him. And I think it's a very common thing, not only with names but also with words that we don't get to use every day.
