In some cases they truly believe they are. For instance, if they have an eating disorder, or just poor self image. I'm in recovery from anorexia. I was so tiny my ribs were sticking out, it hurt to sit down, and I couldn't lay in bed without hurting because all my bones would dig in... Yet I still looked in the mirror and saw someone huge. I felt fat. They may be serious when they say it. Rant done.
