Ugh, I hated when that happened with Avatar: The Last Airbender. They left us with the biggest cliffhanger at the end of season 2, and then there was a huge 9 month hiatus until season 3 started up again. It was torture T__T
It also sucks when it happens in the MIDDLE of the season, as is the case of FlashForward, V, and once again, Avatar (yet ANOTHER cliffhanger in the middle of season 3, followed by a hiatus)
For me it's Grey's Anatomy- it's turned into a soap opra, which I usually hate, but I can't stop watching it, even after my favorite character died :'(

Sherlock slapped us with the hugest cliffhanger ever known....and made us wait one and a half years.

Yeah, it depends. I think the longest I've ever had to wait was five months, but usually for my shows it's not much more than two...
The longest was a year and about 2 weeks for book 2 of Legend of Korra.
