The doctor said that if he doesn't recover,, they'll have to lobotimize him (remove his brain). It's sad and disturbing :(

Umm a Lobotomy does not kill you. They stick a pick up your nose and remove some fibers in your frontal lobe, it makes you have no emotions-but it does not kill you. Thats what he meant by "its better to die a good man than a monster" as in, when he does actually die-he will have no recollection of what he did. They said it created ghosts because It makes you an empty shell of a person, and some would think thats worse than dying. It was popular back in the day, people had their children lobotomized if they acted bad or even had ADD/ADHD. Just saying He technically did not die in shutter island.

He did not die in Shutter Island, a lobotomy wont kill you. Also nobody seem to have seen The Beach-which is a great movie btw, he gets his ass kicked a bit but he doesnt die
