with the 6 billion people on the earth, the chances you would meet AND fall in love/get married with a sibling would be very small

Yes, but not even adopted child comes from overseas - a lot of time, people adopt a child from their own state. It's not a stretch then.
Same deal with IVF - it's usually sperm donated from a local clinic.

They keep records (if the birth parents prefer not to be named, usually a serial number) just for that reason.
And, anyone who was adopted or conceived using donor sperm will tell you that that is at the back of their minds when they meet a potential spouse, and will usually ask about their history too.
Like, in rare cases it does happen, but someone did foresee this, and put measures in place.

That's happened before. They were twins seperated at birth, just happened to come across each other, fall in love, and get married.
