It's the Doctor JUST the Doctor And then the running gag is; "Hi, I'm the Doctor." "The Doctor? Doctor who?"

Actually, in one of the original serials (The Faceless Ones, I believe), the Doctor signs his name as "Doctor W." Not to mention the classic serials/episodes such as "Doctor Who and the Silurians" and "The Death of Doctor Who," as well as ALL of the Target novelizations from May 2, 1973 to August 19, 1982, which were titled "Doctor Who and the..." (with the exception of "The Three Doctors"), leaving the implication that he can, in fact, be referred to as 'Doctor Who.' As a Doctor Who fan, I am bugged when other fans DON'T acknowledge this. ...That being said, for most fans, what you've said is entirely true.
