I've theorized it's a dent or a burn since it came from a dodgeball incident. When he was really little a dodgeball might've been thrown at his head and hit him directly or scraped his head harshly, thus he wears the hat all the time and when it was taken off once Ed (or Eddy I forgot which) says "Does it hurt?"

I saw something online awhile ago about that; apparently in one of the E&E&E computer/video games he throws his hat in the air after you win the level, I'll try to find it. Granted, that is just a game and not an aspect revealed in the show, but still.

I guess the creators just leave it to the fan's imagination. An example being G.I.R. because they never officially told as far as I know but from what I have seen a lot of the fans have just agreed that his name is Garbage-Issued Information Retrieval unit.
