I feel like this should rephrased as "No shirt nor shoes, no service." The way it's listed makes me think it must be a combination of no shirt and no shoes to make deserve no service.

Well yeah. In most of those places, wearing pants is required by law but wearing a shirt or shoes is a matter of a store's preference.
