You also have to realize at that time in America, the idea of a man being r*ped by another man was absolutely so far beyond anyone's ken. It was inconceivable that could happen.
Also, pre-internet so no spoilers were mass spread, so it was pretty much word of mouth for if the movie was good or not, so the anticipation of what was going to happen (will he be, won't he be) was the "thrill" part of the movie.
Also, the "native" characters fit a lot of stereotypes, true, but also at that time in America, there WERE a lot of "Hill billies" that fit the stereotype portrayed.
It was a product of its time.

One of those slow pace movies with a build-up before the payoff and easier to be engaged and pulled in for the ride on a big theater screen
