I think there are different forms of respect. For example, there is a basic form of respect for a person's humanity, and that is unearned. There are also forms of respect that are context-specific and earned. For example, I might respect you as a coworker, but you have to earn that respect by performing well over time.

Due to the fact the primary definition of respect is admiration, I have always and exclusively said and understood the phrase in question to mean: "admiration is earned not given". And this is because people not worthy of admiration often wish to be treated with admiration and not merely with regard.
